Message from the Nurse:

"Hello, my name is Selinda Meadows and I have been with LISD since 1996. I have spent that time on the LHS campus. I have experience in Hospital, Nursing Home, Home Health and Adult Day Care. I have loved all aspects of nursing but I must say I have really enjoyed being a School Nurse. Thank you for sharing your children with us."

Clinic Policies:
• All daily medicine must have paperwork signed by your doctor on file. You may download the medication form below.

• Each year, you can send over-the-counter medicine to school for your student with a signed medication form, available below. Parents are responsible for picking up medicine at the end of the school year. Any medication left after the last day of school will be destroyed. LHS does not furnish medications for students.

• Students may carry inhalers with the proper paperwork. Stop by or call the clinic.

• Please notify the clinic of any medical changes in your student (i.e. surgery, major injuries, changes in medication, etc.) and maintain up-to-date address and phone number information with the registrar. (435-8038)

Immunizations:
Letters will be mailed home 1-2 months before your student has an immunization due. Please contact the clinic in writing or by phone if your student has already received the immunization required. Students will be excluded from school (un-excused absences) if they do not meet the state requirements for immunizations.

Diabetics:
Diabetics need to provide the clinic with a copy of your physician ordered diabetic plan. Communication with the Nurse is very important.
