Proof of immunization
is mandatory for all out of state registrants.
State law prohibits enrollment until proof is provided.
Immunization
Requirements for Students
- All students under the age
of seven must have a minimum of four doses of DPT/DTAP,
with one dose being on or after the fourth birthday.
Students age seven and older must have a minimum of
three doses of DTP/DTAP, with one dose being on or
after the fourth birthday. A booster is required every
10 years.
- All students must have a minimum
of three doses of Polio Vaccine, with one dose being
on or after the fourth birthday. OPV is not required
after age 18.
- All students must have one
dose of Measles, Mumps, and Rubella Vaccine on or after
the first birthday.
Students born on 09-01-91 or before must have a second
dose of Measles Vaccine by their 12th birthday.
- All students born on or after
09-02-91 must have a second dose of Measles Vaccine
before entering school at age five.
- All Pre-Kindergarten students
must have their second dose of Measles Vaccine as they
reach their fifth birthday during the school year.
- All students born on or after
09-02-92 must have completed the series of three Hepatitis
B Vaccine before entering school.
- All students born on or after
09-02-88 but before 09-02-92 will be required to show
proof that they have received three doses of Hepatitis
B Vaccine by their 12th birthday.
- All students under the age
of five must have had one dose of Hib Vaccine after
the age of 15 months or a series and booster completed
by 15 months.
- All students entering Kindergarten,
Pre-Kindergarten, or PPCD must have had one dose of
Varicella (Chickenpox) Vaccine after the age of one
year.
- All students born on or after
09-02-88 but before 09-02-94 must show proof of one
dose of Varicella (Chickenpox) Vaccine after the age
of one year, by their 12th birthday.
- These requirements are effective
August 1, 2000.
